- All participants must submit a waiver prior to joining a club sport. Access the Waiver
- All club sports volunteer coaches/instructors are required to fill out coaching agreement form annually. Download and Complete the Agreement
- The driver’s agreement and injury report forms can be found on the NSU Club Sports SharkHub page.
- The Recreation and Wellness Department for Club Sports uses a tier classification system to allocate club sports funds among all active clubs. Learn More about This System
